Health Care Worker (HCW) Based in Gippsland, Latrobe Regional Health (LRH) are looking for a Health Care Worker to join our Hospital in the Home/Post Acute Care on a casual basis. LRH commenced Home and Personal Care services for Post Acute Care Program in 2023, from previous outsourced model. The HCW is responsible for provision of personal and home care to support people post discharge from acute care, additional same services are provided for Transitional Care program and Hospital in the Home. HCW on duty are supplied with multiple resources car, phone, safety call and cleaning products.
