Position Summary Be part of a team who are focused on providing the highest quality patient-centred care Enhance your career with the largest regional healthcare service in Victoria Temporary Part Time 64 hours per fortnight (until 30/06/2025) About the facility The Residential in Reach program aims to prevent avoidable transfer to the Emergency Department for residents in Aged Care facilities. The Program provides an outreach service to residential aged care for assessment/intervention when the regular general practitioner is unavailable, the resident requires medical attention and is at risk of transfer to ED. The program is a team of clinicians, including Geriatrician, Nurse Practitioner/candidate, roster GP's and Geriatric Medicine Registrars. About the role The role is responsible for providing administration support to the clinicians in the program, including appointment scheduling, entering episodes of care onto clinical systems and general administration support to the team. Work will include liaising with staff within the team and other services, such as GP's and aged care facilities.
