Exciting opportunity exists for Social Worker (Level 1/2) to join our well-respected acute care and rehabilitation team at Ryde Hospital. This 12-month temporary position provides an opportunity for an enthusiastic clinician to expand their skill and experience in areas of acute care including but not limited to psychosocial assessments, crisis intervention, end of life issues, domestic violence, complex discharge planning and rehabilitation services with focus on motivational counselling, NDIS and complex discharge planning. This is unique opportunity to gain experience in acute care team 3 days a week and our rehabilitation team 2 day a week. Where you'll be working The position sits within the Social Work department at Ryde Hospital. Monday to Wednesday the position will be based in acute care team and Thursday to Friday in Graythwaite Rehabilitation centre. This position is part of wider multidisciplinary team providing acute and subacute care to patients at Ryde Hospital.   Ryde Hospital is having an incredible $479 million complete hospital redevelopment that will transform the delivery of healthcare for the Ryde local community. The Ryde Hospital Redevelopment will result in new health facilities and enhanced services including emergency, ICU/critical care, inpatient units, outpatient/ambulatory care services and much more. Construction commenced in early 2023, and will provide you with a wonderful and exciting opportunity to be part of the Ryde team to change the future health services for the Ryde Community completely. What you'll be doing This unique role provides an opportunity to provide frontline social work services to our most vulnerable patients and their families across acute and subacute settings. The successful applicant will provide Social Work interventions including psychosocial assessments, crisis interventions, counselling, advocacy, complex discharge planning to inpatients and their families/carers. Social Work degree and eligibility for the Australian Association of Social Workers (AASW), including the ability to demonstrate a practical application of the AASW Code of Ethics and current NSW Driver Licence. Proven ability to work independently and to establish effectively collaboratively working relationships within a multi-disciplinary team and interagency networks. Demonstrated well developed interpersonal, written and verbal communication skills with the ability to communicate with internal and external stakeholders. Commitment to participation in professional development and quality improvement activities. Demonstrate skills and knowledge in the provision of service to consumers, carers and families requiring assessment, counselling, crisis management, case management, advocacy, intervention, appropriate referral and discharge planning in acute rehabilitation settings. Demonstrated ability to prioritise and manage workload demands, meet deadlines and complete activities (including ability to work within scope of practice and seek assistance when required).
