About the role South West Healthcare's (SWH) Ngootyoong Prevention and Recovery Centre (Ngootyoong PARC) is seeking a Social Worker Care Coordinator. This role is responsible for providing a high level of clinical expertise in the holistic care and management of consumers with complex mental health needs, and their families and carers. The position will have an important role in coordination between the Ngootyoong PARC team and the Community Mental Health and Wellbeing teams and other organisations involved in a consumer's care. This is predominantly a business hours role however some after-hours flexibility may be required to allow for engagement of carers on an individual or group basis. Ngootyoong PARC offers recovery oriented treatment for people with, or at risk of developing, serious mental illness in a home-like setting. PARC is a supported recovery orientated mental health residential facility which provides an option for people who are becoming unwell or who are in the early stages of their recovery and would benefit from short term (up to 28 days) or medium term (less than 6 month) intensive treatment to strengthen their biological, psychological and social well-being. A two week day program is also offered. Permanent full time (80 hours per fortnight) with a monthly ADO position available. This position is eligible for an incentive payment for rent, childcare, relocation costs and more, as part of the new workforce and incentive grants program administered by Rural Agency Workforce Victoria (RWAV). What you bring As a motivated Social Worker with sound clinical knowledge and an understanding of recovery oriented care, you will have demonstrated experience and skills in delivering a broad range of treatment and care coordination to consumers in a mental health environment, preferably in a community setting.
