An exciting opportunity is available for a Social Worker to join the Aged Care team at Nepean Hospital on a permanent full time basis. In this role you will be responsible for providing a high quality social work service to the Aged Care wards at Nepean Hospital, this requires working knowledge of social work theories which inform your practice. You will also be required to work closely with the multiple disciplinary teams (MDT) on the ward/s to achieve best outcomes for our patients in an efficient and effective way. Members of our Aged Care Social Work team are required to complete psychosocial assessments, attend case conferences, facilitate family meetings, provide varying types of counselling and contribute to case and discharge planning. Selection Criteria to be Addressed : Tertiary Qualification in Social Work and eligibility for membership with the Australian Association of Social Workers (AASW) Demonstrated understanding of Aged Care Social Work within a Hospital environment Demonstrated high level communication, report writing and computer skills Demonstrated ability to work independently and as a part of a Multidisciplinary team. Demonstrated understanding of psychosocial stressors that could impact on the admission. Basic knowledge of My Aged Care, Carers Gateway, NSW Civil Administrative Tribunal - Guardianship Division, National Disability Insurance Scheme, the Ageing and Disability Commission and Residential Aged Care. Demonstrated capacity to Link theory and practice Current unrestricted driver's license and willingness to travel for work.
