The Sutherland Transitional Aged Care Service (STACS) is the opportunity you have been looking for Where you'll be working The Sutherland Transitional Aged Care Service (STACS) is a multidisciplinary team working out of the newly multimillion dollar renovated Southcare building. STACS services the Sutherland Shire delivering rehabilitation in the home environment post-acute admission to hospital, to patients > 65 years of age and assessed as eligible to receive services by ACAT. What you'll be doing The STACS Level 1-2 Social Worker is an integral member of the multidisciplinary team. The Level 1-2 is supported by a Level 3 Social Worker and provides mentorship and clinical leadership to the Level 1-2. Your role with in STACS is to provide psychosocial assessment, service establishment and crisis situation management to patients on the STACS program. Plan, coordinate, and implement in-depth levels of clinical expertise in Social Work and provide high quality Social Work service to clients of the Sutherland Transitional Aged Care Service (STACS) consistent with Australian Association of Social Workers, NSW Health, South Eastern Sydney Local Health District (SESLHD) policies, procedures and standards. Employment of a temporary visa holder may only occur if no suitable permanent resident or citizen of Australia has been identified for this position following suitable labour market testing. All workers and new recruits are required to receive 2 doses of a Therapeutic Goods Administration approved or recognised COVID-19 vaccine to commence employment/ engagement or continue to work within a NSW Health service.   A worker and new recruit will be considered compliant if they have a medical contraindication to all available Therapeutic Goods Administration approved or recognised COVID-19 vaccines and provide medical contraindication evidence in line with the policy requirements. In addition, all Category A workers and new recruits are required to receive one dose of the seasonal influenza vaccine annually to be considered compliant. Category A workers and new recruits who are non-compliant with seasonal influenza vaccination or have a medical contraindication to influenza or COVID-19 vaccinations must comply with all other infection control risk reduction strategies as directed while working in a Category A position. Current unrestricted Class C NSW driver's license. Post Graduate Social Work clinical experience demonstrating competencies in comprehensive assessment and case management of frail older clients with complex health problems and other community support needs. Demonstrated comprehensive knowledge of community aged care resources. Demonstrated ability to work independently and as part of a multidisciplinary team. Demonstrated organisational skills and ability to manage a demanding caseload. Demonstrated high level communication, report writing skills and computer skills.
