What we can offer you (for eligible employees): •    Accrued Day Off (ADO) (for full time employees) •    Opportunity for extra tax savings through Salary Packaging  •    Novated Leasing •    Great education opportunities through Education Training Service which offers over 110 courses each year  •    Access to our Employee Assistance Program (EAP) for staff and family members •    Fitness Passport What you will be doing:  An exciting opportunity exists for a Senior Social Worker to join our Substance Use in Pregnancy and Parenting Service (SUPPS) which is located at St Marys Community Health Centre.   Your role as a Senior Social Worker working with the Substance Use in Pregnancy and Parenting Service (SUPPS), will provide a continuum of care to pregnant women and their children during the antenatal and postnatal periods within a community setting. You will also provide specialist counselling and case management support services for patients who use drugs or alcohol and are pregnant or responsible for parenting children under the age of two years old.  The core functions of this position are:  Develop linkages and referral pathways across a range of community-based support services to address issues such as child protection, harmful patterns of substance use, relationship difficulties, maternity and women’s health, early childhood, domestic violence, housing and legal problems Promote the principles of Keep Them Safe (child protection and well-being), harm minimisation and a recovery model of care from substance related issues Develop clinical policies and procedures for the smooth running of the team in consultation with managers and other staff Facilitate and participate in interagency child protection meetings and case conferences with patients and their families or significant others Under the direction of a medically led multi-disciplinary team, perform specialist clinical services to patients including comprehensive drug and alcohol assessments, monitor and appropriately respond to concerns around safety of patients and their children (including violence abuse and neglect issues), to work collaboratively with patients to develop a comprehensive care plan and review case plans at regular intervals or sooner if required Provide assertive follow up for patients including sustained home visiting of clients deemed to be at high risk for placement of children in Out Of Home Care services Deliver education to other staff of the Drug and Alcohol Service and implement quality improvement programs Provide supervision and support to team members An eligibility list may be created for future vacancies.  About us: Nepean Blue Mountains Local Health District (NBMLHD) is a wonderful place to expand your career and grow your skills and knowledge. Booster doses are highly recommended for all health care workers who have completed the primary course of COVID-19 vaccinations. Employment of a temporary visa holder may only occur if no suitable permanent resident or citizen of Australia has been identified and must not exceed the duration or conditions associated with the current visa. Selection criteria to be addressed: Tertiary qualifications in Social Work and eligibility for membership with the Australian Association Social Workers (AASW). Extensive post-graduate clinical experience in drug and alcohol counselling as well as in an area such as child protection, domestic violence and/or family relationship issues Demonstrated experience with using advanced reasoning skills including the preparation of high quality written and oral reports, and comprehensive care plans Demonstrated knowledge of evidence-based psychosocial interventions for addressing domestic violence, child abuse or neglect issues and/or harmful patterns of parental substance use. Extensive experience in delivering psychosocial interventions including monitoring of clinical outcomes and responding to risk factors for child protection issues Demonstrated knowledge and extensive experience in maintaining professional boundaries and ethical practice when working with families and other service providers. Previous experience and a commitment to work within a medically led multidisciplinary team. Extensive experience in partnering with parents and families to address child protection and well-being issues including reducing the risks associated with harmful patterns of parental substance use Previous experience with communicating information in a client electronic record system and other computer based applications. Extensive experience in the timely communication and escalation of significant concerns to medical specialists and/or line managers Current Driver's Licence and willingness to use for work purpose travel.
