Senior Social Worker – Grade 3 Sub-acute inpatients Provide expert clinical skills, leadership and knowledge in the sub-acute inpatient services including NDIS, Victorian Civil and Administrative Tribunal (VCAT) hearings, and My Aged Care. Your New Role Provide exemplary social work service in the relevant sub-acute wards (GEM and/or inpatient rehabilitation) including assessment, intervention, and discharge planning. Foster positive team and workplace culture as a senior Social Worker. Utilise specialised skills and knowledge in relation to relevant community providers communicating between patients and caregivers. Demonstrate excellent communication strategies with patients, relatives, staff and relevant services in order to provide appropriate services, information and leadership Actively lead and participate in Allied Health quality improvement initiatives and research About You You have demonstrated hospital experience with a collaborative leadership style You have excellent communication skills and a passion for continued professional development for yourself and others. You are a qualified Social Worker, eligible for membership with the Australian Association of Social Workers. You value working with supportive and dedicated health professionals and multidisciplinary teams.
