About the Role: The incumbent will be accountable to the Team Leader of NALHN (Northern Adelaide Local Health Network) Community Older Persons Mental Health Service (OPMHS). Your role will be to work within the NALHN OPMHS Community Team, providing therapeutic engagement and coordination of care to the range of older people receiving community based mental health services. You will be comfortable working as a member interdisciplinary team, spending time out and about with consumers living in the community. You will also be ready to be part of nurturing the NALHN OPMHS growth culture, built around developing a values-based workforce, facilitating excellence in care, and providing transparent accountability. You will work in partnership with the other members of the team within an aspirational organisational change process. NALHNs purpose is to deliver exceptional health and wellbeing services that enhances the quality of life for our community. The social worker will undertake social work assessment and interventions for consumers with current positions across community. The social worker, in collaboration with the multi-disciplinary team works to optimise health outcomes and to ensure a high standard of consumer-centred care and safe and timely transfer of care to primary health. In this role you will apply clinical experience, knowledge and professional competence to plan, implement and evaluate comprehensive and integrated Social Work services to the needs of consumers of OPMHS. You will work under clinical supervision and may provide direction and support to less experienced staff, allied health assistants and students. You will work collaboratively with the multi-disciplinary team and liaise directly with health services and community agencies. You will be accountable to the Senior Social Worker, for your professional practice and development. Social worker in community will have specialist knowledge in the following areas: Advance Care Directives, Guardianship and Administration matters, Elder Abuse, Domestic Violence, My Aged Care, National Disability Insurance Scheme, and community support services. Social workers serve as care coordinators and will be matched with consumers with significant psycho-social needs. Social Workers provide their discipline related expertise to the multi-disciplinary team. To be eligible for this position, you must have an appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers The South Australian public sector promotes diversity and flexible ways of working including part-time.
